What is HEART VALVE SURGERY or Valve replacement surgery?
Valvular heart disease affects any of the four heart valves. Most commonly mitral valve and aortic valve is affected. Mitral valve disease generally affects younger populations but not very rare in older age group also.
Mitral valve stenosis or mitral valve leakage or mixed pathology ( both stenosis and leakage) are generally caused by rheumatic heart disease in India. The patient s will generally present with breathlessness, chest pain or palpitations. In advanced disease heart failure and death can occur within 5 to 7 years. Mitral valve replacement and repair surgery in these patients can be life saving .
If not repairabl, valve replacement is the only choice. BMV can be done in very selective patients in very early stage of mitral stenosis without any significant coexisting leakage in the valve. Post BMV restenosis can occur in variable time.Mitral valve replacement with mechanical or bio prosthetic valve is a simple procedure with good outcome. In case of mechanical valve replacement the valve will last life long . An oral anticoagulant is prescribed in these patients and periodic checkups for proper action of the drug ( INR) is recommended.

Heart Valve Repair or Replacement Surgery
Heart valve surgery is open-heart surgery through the breastbone, into the chest. It is a major operation that can last two hours or longer and recovery often takes several weeks
For bioprosthetic valve replacement the oral anticoagulant is not recommended and no periodic blood test is required. The bioprosthetic valve generally lasts for 10 to 15 years.
Aortic Valve disease like aortic valve stenosis and regurgitation in younger age group is generally due rheumatic valve disease in India. In older age group this disease is due age related degenerative disease.
Aortic valve replacement in severe aortic stenosis and aortic regurgitation is safe surgery.
The valve choice and recommendations are similar like mitral valve replacement.
Double valve replacement is indicated when both aortic and mitral valve is severely damaged.
Tricuspid valve disease generally coexist with mitral valve disease. Tricuspid valve repair can be done along with mitral valve procedure. Very rarely tricuspid valve replacement is indicated.
Pulmonary valve disease is generally a pediatric and neonatal heart disease. Very rarely replacement is needed. For isolated pulmonary valve stenosis either a open or a closed valvotomy is indicated
